The transsylvian approach to middle cerebral artery bifurcation/trifurcation aneurysms.

چکیده
A transsylvian approach to treat aneurysms located at the bifurcation/trifurcation of the middle cerebral artery (MCA) is described. This surgical route traces a distal MCA branch retrogradely into the Sylvian fissure whereupon a secondary MCA branch and the main stem of the MCA (M1) are identified before aneurysm clipping. The advantages, and disadvantages of this approach are discussed. The transsylvian route should be considered as an alternative to the traditional pterional approach, which utilizes proximal to distal dissection of the MCA, in situations when the M1 is long or when the pterional approach is inadvisable.

Background: This investigation was conducted in order to study angiographic findings in patients with cerebral aneurysm. Materials and Methods: The study conducted on 136 cases of ruptured cerebral aneurysms between 1995-2000 confirmed by means of 4-vessel cerebral angiography to get an insight to racial, geographic and environmental factors predisposing to the occurrence of subarachnoid hemorr...
متن کاملBranching pattern of middle cerebral artery in an African population.
Branching pattern of middle cerebral artery influences frequency of its aneurysms, and is of potential value in their surgical repair and diagnosis of stroke. This pattern shows inter-population variations but there is paucity of data from Africans. This study aimed at describing branching pattern among black Kenyans. Middle cerebral arteries numbering 288 from 144 formalin fixed brains obtaine...
متن کاملMicrosurgical and angiographic anatomy of middle cerebral artery aneurysms: prevalence and significance of early branch aneurysms.
OBJECTIVE To determine the prevalence of early branch aneurysms, characterize these lesions angiographically and anatomically, and determine their clinical significance. METHODS The authors conducted a retrospective review of 125 consecutive patients with a diagnosis of middle cerebral artery (MCA) aneurysm. Eighty-four patients harboring 100 MCA aneurysms were studied; 41 patients were exclu...
متن کاملSuperficial Sylvian veins as landmarks for operation of middle cerebral artery aneurysms.
A retrograde approach to aneurysms located at the middle cerebral artery bifurcation which uses the superficial Sylvian veins as landmarks is described. Fifty patients with 51 aneurysms were operated on using this technique. This method made it easier for surgeons to decide where to enter the Sylvian fissure for dissection of the middle cerebral artery branches and to secure the parent artery.
متن کاملEndovascular and Surgical Options for Ruptured Middle Cerebral Artery Aneurysms: Review of the Literature
Middle cerebral artery (MCA) aneurysms are common entities, and those of the bifurcation are the most frequently encountered sublocation of MCA aneurysm. MCA bifurcation (MBIF) aneurysms commonly present with subarachnoid hemorrhage (SAH), are devastating, and are often lethal. At the present time, the treatment of ruptured MBIF aneurysms entails either endovascular or open microneurosurgical m...
